During a bug regression test, the 2188 Tetsuya Ida noticed that a part of the final phase had been altered, so he reported this change to Renya Gouto. The change was made in Chihiro Morimura's host to trigger a memory overwrite. Though the memories' contents remain unknown, it's clear that Juro Izumi was the one who made the change.[1]
